Join the Alliance for Home Health Quality and Innovation for a discussion of the Future of Home Health Care Framework, including the release of case studies and appendices which will inform the upcoming independent publication. The report is the culmination of a four phase research-based strategic planning project on the future of home health care in America.

As Americans age and live longer with multiple chronic conditions and incresed difficulties with daily living, our nation is facing a health care challenge to ensure that the majority of older Americans with serious chronic illness and other age related difficulties can remain as independent as possible in their homes and communities. The success of this challenge has tremendous implications for a rapidly growing population of older Americans and the viability of the Medicare and Medicaid programs.

Leading experts and stakeholders will provide an overview of the report and share an initial vision to support the transformation of home health and home-based care as an integral part of the overall health care delivery system.
